1. Proof of Immigration Status for all family members

Resident Card “Green Card” (I-551)

2. Proof of Income for all family members

Tax return

Wages and tax statement (W-2)

Letter from employer

Pay Stubs —4 needed if paid weekly, 2 needed if paid biweekly, and 1 needed if paid monthly

3. Proof of Identity for all family members

Driver’s license issued by state or territory

Identification card issued by the federal, state, or local government

4. Social Security number for all family members
