Bring the following items to your appointment.
1. Proof of Immigration Status for all family members
- Resident Card “Green Card” (I-551)
2. Proof of Income for all family members
- Tax return
- Wages and tax statement (W-2)
- Letter from employer
- Pay Stubs —4 needed if paid weekly, 2 needed if paid biweekly, and 1 needed if paid monthly
3. Proof of Identity for all family members
- Driver’s license issued by state or territory
- Identification card issued by the federal, state, or local government
