Maintaining the confidentiality of medical information and counseling records is of vital importance to us. All client information is kept confidential within this organization, except where there is threat of physical harm (i.e. homicide, suicide, child abuse, or child neglect), which the law requires us to report. Medical records are only released to the patient. Patients must sign a Release of Records form before records will be released to another party.
